After Hours Facility

 Meadowland Medical Practice understands the importance of providing continued care to our regular patients after hours. 

To ensure continuity of care, our practice offers home visits to our regular patients in their homes, aged care facilities, and residential care facilities. These visits are made within and outside of normal working hours, provided it is safe and reasonable to do so.

In situations where home visits are not safe or reasonable, our policy outlines an alternate system of care for patients who require this service.

Our practice provides home visits only in special circumstances.

Additionally, we collaborate with Hello Home Doctor Services, an accredited medical deputizing service, to offer home visits. Our partnership with Family Care guarantees:

• Next-day reporting to the Practitioner (via fax) of the care provided to the patient.
• Phone contact access for the deputizing practitioner to patient health information in exceptional circumstances.

These services are delivered by appropriately qualified health professionals.
